This page explains each stage of your ADHD assessment journey with Lycali, from initial enquiry and registration through to questionnaires, informant information, assessment, report, feedback and recommendations.

Initial Enquiry & Registration
Once you contact Lycali, our team will review your enquiry and provide information about the most appropriate assessment pathway based on your needs.
If you decide to proceed, you will be sent a registration form to complete. This allows us to gather key information about you and register you securely on our clinical management system.
The registration form includes information such as your contact details, GP details, relevant background information and consent preferences.
Invoice, Terms & Conditions and Consent
Once registered, we will issue your assessment invoice together with our terms and conditions and consent documentation.
These documents must be reviewed and signed before your assessment can proceed.
Payment Received
Once payment has been received and all documentation has been completed, your assessment pathway will formally begin.
You will then receive access to the required pre-assessment questionnaires and any additional screening documents needed for your assessment.
Documents to Complete
Please complete the forms below before your appointment. These help us gather information about your developmental history, attention, organisation, executive functioning, emotional regulation and day-to-day experiences.
Adult Developmental History Questionnaire
This questionnaire helps us understand your developmental history, childhood experiences, education, relationships, communication profile and life journey.
Adult ADHD Add-On Questionnaire
This questionnaire gathers additional information about attention, concentration, organisation, impulsivity, hyperactivity, emotional regulation and daily functioning.
Additional Screening Questionnaires
If you have already completed an ADHD pre-screening assessment with Lycali, you will not usually need to complete these questionnaires again unless requested by a clinician.
If you have not previously completed a pre-screening assessment, you may be asked to complete additional screening questionnaires, including:
- Adult ASRS
- Adult SDQ
- Wender Utah Rating Scale (WURS)
These questionnaires will be sent separately to your email address. Please check your inbox, junk mail and spam folders regularly to ensure you receive all assessment documentation.
Informant Questionnaire
As part of the ADHD assessment process, we ask for information from someone who knows you well. This helps us gain a fuller understanding of your childhood development and current day-to-day functioning.
Suitable informants may include:
- A parent who can provide information about your childhood
- A partner or spouse
- A sibling or close family member
- A close friend who has known you for a significant period of time
Please ask your chosen informant to complete the questionnaire below.
Assessment Appointment
Your ADHD assessment will be completed by an appropriately qualified clinician and will consider information gathered from questionnaires, screening measures, developmental history and informant information.
The assessment includes a structured clinical interview exploring:
- Attention and concentration
- Organisation and planning
- Impulsivity
- Hyperactivity or restlessness
- Emotional regulation
- Education and employment experiences
- Relationships and social functioning
- Daily living skills and executive functioning
The adult being assessed should attend the appointment. Where appropriate, an informant may also attend or provide information separately to support the assessment process.
Report, Feedback and Recommendations
Following the assessment, the clinician will review and analyse all information gathered throughout the process.
You will receive a comprehensive written report summarising:
- Assessment findings
- Clinical observations
- Information gathered from questionnaires and informants
- Diagnostic conclusions where appropriate
- Strengths and areas of need
- Recommendations and next steps
Recommendations may include workplace adjustments, reasonable adjustments, coaching, therapy, psychoeducation, Access to Work support, medication referral pathways and post-diagnostic resources.
What Happens After Diagnosis?
Following assessment, you may wish to access additional support, coaching, therapy, workplace adjustments, psychoeducation or ADHD-specific resources.
