Child ADHD Assessment – Required Documents

To help us prepare for your child's ADHD assessment, please complete the steps below as soon as possible. Early completion of the required documentation helps us gather information, contact your child's educational setting, and avoid delays in the assessment process.

Step 1: Complete the Child Developmental History Questionnaire

This questionnaire provides essential information about your child's developmental history, communication, behaviour, education, health, family background, and day-to-day functioning.

Please complete this questionnaire as soon as possible following booking.

We also require details of your child's current educational setting so that we can request information from school, nursery, college, or other educational providers where appropriate.

If there is likely to be a delay in completing the Developmental History Questionnaire, please email info@lycali.co.uk with your child's educational setting details as soon as possible to avoid delays in the assessment process.

Step 2: Complete the Child ADHD Add-On Questionnaire

This questionnaire gathers additional information regarding attention, concentration, organisation, impulsivity, hyperactivity, emotional regulation, and day-to-day functioning.

Step 3: Educational Setting Questionnaire

We ask that a member of staff from your child's educational setting completes this questionnaire. This may be completed by a teacher, SENCo, key worker, pastoral lead, teaching assistant, or another member of staff who knows your child well.

The information provided helps us understand how your child presents within the educational environment and forms an important part of the assessment process.

Please share the questionnaire below with the relevant member of staff.

Step 4: Additional Screening Questionnaires

If your child has already completed a Lycali ADHD Pre-Screening Assessment, these questionnaires will not usually need to be completed again unless specifically requested by the assessing clinician.

Where required, you may be asked to complete the following questionnaires:

  • AQ-50 (Parent to Complete)
  • SNAP-IV 26 (Teacher or Parent to Complete)
  • Strengths and Difficulties Questionnaire (SDQ)

These questionnaires will be sent separately to your email address. Please check your inbox, junk mail and spam folders regularly to ensure you receive all assessment documentation.

In some cases, copies may also be sent directly to your child's educational setting where appropriate.

If you have not received these questionnaires within 5 working days of booking your assessment, please contact the Lycali team.

Step 5: Young DIVA Parent Interview

As part of the ADHD assessment process, we complete a detailed Young DIVA interview with a parent or caregiver who knows the child's developmental history and current functioning well.

This appointment explores ADHD-related symptoms across childhood and current functioning, including attention, organisation, impulsivity, activity levels, emotional regulation, education, friendships, and daily life.

Wherever possible, we encourage the child or young person to attend for part or all of this appointment, as this can provide valuable additional information.

Where appropriate, both parents may attend together.

This appointment forms an important part of the ADHD assessment process.

The Lycali administration team will contact you to arrange a suitable appointment time.
