Child Dyscalculia Assessment – Required Documents

To help us prepare for your child's dyscalculia assessment, please complete the steps below as soon as possible. Early completion of the required documentation helps us gather information, contact your child's educational setting, and avoid delays in the assessment process.

Step 1: Complete the Child Developmental History Questionnaire

This questionnaire provides essential information about your child's developmental history, education, learning profile, communication, health, family background, and day-to-day functioning.

Please complete this questionnaire as soon as possible following booking.

We also require details of your child's current educational setting so that we can request information from school, college, or other educational providers where appropriate.

If there is likely to be a delay in completing the Developmental History Questionnaire, please email info@lycali.co.uk with your child's educational setting details as soon as possible to avoid delays in the assessment process.

Step 2: Complete the Dyscalculia Add-On Questionnaire

This questionnaire gathers additional information regarding mathematical development, number concepts, calculation skills, memory, processing speed, learning experiences, educational support, confidence with maths, and day-to-day functioning.

Step 3: How I Feel About Mathematics Questionnaire

This questionnaire is completed by the child or young person (where age appropriate) and helps us understand their experiences of mathematics, confidence levels, emotional responses to maths, and how mathematical difficulties may be impacting everyday life and education.

This questionnaire is suitable for children and young people aged 8–18 years.

Step 4: Educational Setting Questionnaire

We ask that a member of staff from your child's educational setting completes this questionnaire. This may be completed by a teacher, SENCo, maths lead, learning support lead, teaching assistant, or another member of staff who knows your child well.

The information provided helps us understand your child's mathematical abilities, classroom performance, strengths, challenges, and support needs within the educational environment.

Step 5: Dyscalculia Assessment Appointment

The assessment appointment will involve a detailed discussion regarding your child's developmental history, educational experiences, strengths, and areas of difficulty with mathematics and numeracy.

A range of standardised assessments will be completed to explore mathematical reasoning, number concepts, calculation skills, working memory, processing speed, and other areas relevant to dyscalculia.

Please note: Dyscalculia assessments are completed face-to-face only.

Due to the nature of the assessment materials and standardised testing procedures, dyscalculia assessments cannot be completed remotely.

Parents or carers will usually be asked to attend part of the appointment to provide additional information where required.

The Lycali administration team will contact you to arrange a suitable appointment time.
