Initial Enquiry & Registration
Once you contact Lycali, our team will review your enquiry and provide information about the most appropriate dyslexia assessment pathway for your child.
If you decide to proceed, you will be sent a registration form to complete. This allows us to gather key information about your child and register them securely on our clinical management system.
The registration form includes information such as parent/carer contact details, GP details, relevant background information and consent preferences.
Invoice, Terms & Conditions and Consent
Once registered, we will issue your assessment invoice together with our terms and conditions and consent documentation.
These documents must be reviewed and signed before your child's assessment can proceed.
Payment Received
Once payment has been received and all required documentation has been completed, your child's assessment pathway will formally begin.
You will then receive access to the required pre-assessment questionnaires and any additional documents needed for the assessment.

Please complete the forms below as soon as possible. These help us gather information about your child's developmental history, education, communication, learning profile, health, family background and day-to-day functioning.
Child Developmental History Questionnaire
This questionnaire provides essential information about your child's developmental history, education, communication, learning profile, health, family background and day-to-day functioning.
We also require details of your child's current educational setting so that we can request information from school, college or other educational providers where appropriate.

Dyslexia Add-On Questionnaire
This questionnaire gathers additional information regarding literacy skills, reading, spelling, writing, memory, processing speed, organisation, learning experiences, educational support and day-to-day functioning.
Visual History Questionnaire
This questionnaire helps us gather information about visual difficulties, visual comfort, reading fatigue, tracking difficulties, light sensitivity and other visual experiences that may impact learning.
This information helps us consider whether any visual factors may be contributing to your child's learning difficulties and whether further investigation may be beneficial.
Educational Setting Questionnaire
We ask that a member of staff from your child's educational setting completes this questionnaire. This may be completed by a teacher, SENCo, learning support lead, pastoral lead, teaching assistant or another member of staff who knows your child well.
The information provided helps us understand your child's learning profile, literacy skills, classroom performance, strengths, challenges and support needs within the educational environment.

Dyslexia Assessment Appointment
The assessment appointment will involve a detailed discussion regarding your child's developmental history, educational experiences, strengths and areas of difficulty.
A range of standardised assessments will be completed to explore literacy skills, reading accuracy, reading fluency, spelling, phonological processing, memory, processing speed and other areas relevant to dyslexia.
Your child should attend the assessment appointment. Parents or carers will usually be asked to attend part of the appointment to provide additional information where required.
The Lycali administration team will contact you to arrange a suitable appointment time.
Report, Feedback and Recommendations
Following completion of the assessment, the clinician will review and analyse all information gathered throughout the process.
You will receive a comprehensive written report summarising:
- Assessment findings
- Standardised assessment results
- Information gathered from parents/carers and educational settings
- Strengths and areas of need
- Diagnostic conclusions where appropriate
- Recommendations and next steps
Recommendations may include school adjustments, literacy strategies, assistive technology, exam access considerations, home support strategies and further support options.
What Happens After Diagnosis?
Following assessment, you may wish to access additional support, school recommendations, literacy intervention, assistive technology guidance, exam access advice or practical resources.
