Psychoeducation, Coaching & Therapeutic Support Questionnaire

This questionnaire helps us understand your child's emotional wellbeing, current concerns, strengths, goals, relationships, support needs and any challenges they may be experiencing at home, in education or within their community.

The information gathered helps ensure therapy is personalised to the needs of the child and family.

Additional Measures

Depending on your child's age, presenting concerns and therapeutic goals, additional questionnaires may be requested to support the assessment and therapy planning process.

Examples may include:
  • Strengths and Difficulties Questionnaire (SDQ)

  • Anxiety measures
  • Mood and wellbeing questionnaires
  • Parent questionnaires
  • Goal-based outcome measures

Initial Therapeutic Assessment

The first appointment provides an opportunity to understand your child's emotional wellbeing, strengths, current challenges, goals and therapeutic support needs.

The initial assessment may explore:
  • Emotional wellbeing and mental health
  • Anxiety, worries and stress
  • Confidence and self-esteem
  • Friendships and relationships
  • Family dynamics
  • School-related challenges
  • Behaviour and emotional regulation
  • Neurodivergence and self-understanding
  • Therapeutic goals and priorities

Parents or carers will usually attend all or part of the initial assessment appointment. This helps ensure the therapist develops a comprehensive understanding of the child's strengths, challenges and support needs.

Following the assessment, recommendations regarding the most appropriate therapeutic approach and frequency of support will be discussed.

Therapy sessions are personalised to the individual child or young person and may draw upon a range of evidence-based therapeutic approaches depending on their needs and goals.
