Pre-Assessment Questionnaires Through WriteUpp
Parents/carers will be asked to complete questionnaires exploring their child's developmental history, educational experiences, literacy, numeracy, strengths and support needs.
All required questionnaires will be issued through WriteUpp, Lycali's clinical management system.
You will receive the relevant questionnaire links separately. Please complete them as soon as they are issued.
Child Developmental History Questionnaire
This explores your child's developmental background, communication, education, health, family history, learning experiences and everyday functioning.
It also helps us gather details of previous educational support and the current educational setting.
Combined Dyslexia & Dyscalculia Questionnaire
This explores reading, spelling, writing, numeracy, mathematical reasoning, memory, processing speed, learning experiences, educational support, confidence and everyday functioning.
Please set aside enough uninterrupted time to complete each questionnaire carefully in one sitting.
Questionnaires cannot be saved part-way through and cannot be resubmitted once submitted.
Please check your answers before selecting Submit.
Allow Enough Time
Have relevant developmental and educational information available before starting.
Cannot Save Progress
Partially completed questionnaires cannot be saved and resumed later.
One Submission Only
Once a questionnaire is submitted, it cannot be resubmitted using the same link.
If you would like a copy of submitted questionnaire responses, please contact Lycali. We can share them subject to appropriate identity and information-sharing checks.
If you notice an error after submitting a questionnaire, please contact our administration team so it can be addressed.
All required WriteUpp questionnaires, registration documents and consent forms must be completed before the relevant assessment session.
Visual History Information
A Visual History Questionnaire may be requested as part of your child's combined assessment.
This explores visual comfort, reading fatigue, tracking difficulties, light sensitivity, visual stress and experiences that may affect reading and learning.
The Visual History Questionnaire will be issued through WriteUpp.
Please complete it alongside the other pre-assessment questionnaires before your child's appointment.
Visual history information helps the assessor consider relevant visual experiences alongside your child's wider literacy and numeracy profile.
If visual concerns are identified, further assessment by an appropriate eye-care professional may be advised.
A questionnaire does not replace a formal eye examination.
Please allow time to complete the questionnaire in one sitting, as it cannot be saved or resubmitted.
How I Feel About Mathematics
Children and young people may be asked to complete the How I Feel About Mathematics questionnaire as part of the dyscalculia assessment.
This helps us understand their confidence, emotional responses, experiences of mathematics and how mathematical difficulties may affect everyday life.
The questionnaire is designed for children and young people aged 8–18 years.
Your child should complete the questionnaire themselves where age and ability allow, with appropriate assistance if needed.
The questionnaire will be issued through WriteUpp.
Mathematics can sometimes be associated with frustration, reduced confidence, avoidance or anxiety.
Understanding your child's feelings helps us consider their emotional experiences alongside the formal assessment findings.
There are no right or wrong feelings. Please encourage your child to respond openly and in their own words.
Please set aside enough time for your child to complete the questionnaire in one sitting. It cannot be saved part-way through or resubmitted.
Educational Setting Information
As part of your child's combined assessment, we will usually request information from their school, college or other educational setting.
This helps us understand literacy skills, numeracy skills, learning experiences, strengths, classroom performance and support needs.
A teacher, SENCo, learning support lead, maths lead, literacy lead, teaching assistant or another professional who knows your child well.
The Educational Setting Questionnaire will be issued through WriteUpp.
Lycali will arrange for the questionnaire to be issued to the appropriate educational professional where required.
