This questionnaire provides essential information about your child's developmental history, education, learning profile, communication, health, family background, and day-to-day functioning.

Please complete this questionnaire as soon as possible following booking.

We also require details of your child's current educational setting so that we can request information from school, college, or other educational providers where appropriate.

If there is likely to be a delay in completing the Developmental History Questionnaire, please email info@lycali.co.uk with your child's educational setting details as soon as possible to avoid delays in the assessment process.

Step 2: Complete the Combined Dyslexia & Dyscalculia Add-On Questionnaire

This questionnaire gathers additional information regarding literacy skills, reading, spelling, writing, numeracy, mathematical reasoning, memory, processing speed, learning experiences, educational support, confidence, and day-to-day functioning.

Step 3: Complete the Visual History Questionnaire

This questionnaire helps us gather information about visual difficulties, visual comfort, reading fatigue, tracking difficulties, light sensitivity, and other visual experiences that may impact learning.

This information helps us identify whether any visual factors may be contributing to your child's literacy or numeracy difficulties.

Step 4: Complete the "How I Feel About Mathematics" Questionnaire

This questionnaire is completed by the child or young person (where age appropriate) and helps us understand their experiences of mathematics, confidence levels, emotional responses to maths, and how mathematical difficulties may be impacting everyday life and education.

This questionnaire is suitable for children and young people aged 8–18 years.

Step 5: Educational Setting Questionnaire

We ask that a member of staff from your child's educational setting completes this questionnaire. This may be completed by a teacher, SENCo, learning support lead, maths lead, literacy lead, teaching assistant, or another member of staff who knows your child well.

The information provided helps us understand your child's learning profile, literacy skills, numeracy skills, classroom performance, strengths, challenges, and support needs within the educational environment.

Please share the questionnaire below with the relevant member of staff.

Step 6: Combined Dyslexia & Dyscalculia Assessment Appointment

The assessment appointment will involve a detailed discussion regarding your child's developmental history, educational experiences, strengths, and areas of difficulty relating to literacy and numeracy.

A range of standardised assessments will be completed to explore reading, spelling, writing, phonological processing, mathematical reasoning, number concepts, calculation skills, working memory, processing speed, and other areas relevant to dyslexia and dyscalculia.

Please note: The dyscalculia component of the assessment is completed face-to-face only.

Due to the nature of the assessment materials and standardised testing procedures, combined dyslexia and dyscalculia assessments cannot be completed fully remotely.

Parents or carers will usually be asked to attend part of the appointment to provide additional information where required.
