To help us understand your family's circumstances and support needs, please complete the steps below before your initial appointment.

This provides background information about your child, family circumstances, education, health, support needs, and day-to-day functioning.

Step 2: Psychoeducation, Coaching & Therapeutic Support Questionnaire

This helps us understand current concerns, family priorities, support needs, goals, education involvement, and any wider advocacy needs.

Complete Support Questionnaire

Step 3: Initial Advocacy & Functional Support Consultation

During this appointment we will discuss current difficulties, desired outcomes, education, health, social care involvement, support needs, and any ongoing advocacy requirements.
