This questionnaire provides important background information about your child, family circumstances, education, health, support needs, strengths, challenges and day-to-day functioning.

Understanding the wider family context helps us identify areas where additional support, advocacy or practical strategies may be beneficial.

Psychoeducation, Coaching & Therapeutic Support Questionnaire

This questionnaire helps us understand your current concerns, family priorities, support needs, desired outcomes, educational involvement, professional involvement and any wider advocacy requirements.

It also allows us to identify barriers that may be impacting your child or family and helps us tailor support to your individual circumstances.

Initial Advocacy & Functional Support Consultation

The initial consultation provides an opportunity to explore your family's current situation, identify priorities and agree how Lycali can best support you moving forward.

The consultation may explore:
  • Current challenges and concerns
  • Education, health and social care involvement
  • School attendance and participation
  • Communication between professionals and services
  • Access to appropriate support
  • Family wellbeing and practical challenges
  • EHCP, SEN Support or educational concerns
  • Reasonable adjustments and accommodations
  • Future planning and desired outcomes

Parents, carers and other family members are welcome to attend where appropriate.

Advocacy & Functional Support Plan

Following the consultation, we will work collaboratively with you to develop a practical support plan based on your family's priorities and agreed outcomes.

Support may include:
  • Education advocacy
  • EHCP and SEN support
  • Support preparing for meetings
  • Reviewing reports and recommendations
  • Support communicating with schools or professionals
  • Identifying reasonable adjustments
  • Practical strategies for home and community settings
  • Signposting to additional services and resources
  • Transition planning
  • Family support and psychoeducation

Ongoing Advocacy & Family Support

Ongoing support is tailored to your family's needs and may be provided through consultations, meetings, liaison with professionals, practical planning sessions or review appointments.

The level and frequency of support will depend on your family's circumstances and goals.

Our aim is to help families feel confident navigating systems, accessing appropriate support and achieving positive outcomes for their child or young person.
