Child Psychoeducation Support – Required Documents

To help us prepare for your child's psychoeducation support, please complete the steps below before the first appointment.

This provides essential background information about your child's development, education, health, wellbeing, and day-to-day functioning.

This helps us understand your child's emotional wellbeing, support needs, strengths, goals, and areas of difficulty.

You may be asked to complete an SDQ parent and/or child questionnaire, depending on your child's age and clinical need.

If this is required, it will be sent separately by email. Please check your inbox, junk mail, and spam folders.

The first session will focus on understanding your child's strengths, needs, goals, and support priorities.
