Combined Adult Autism & ADHD Assessment – Required Documents

To help us prepare for your combined autism and ADHD assessment, please complete the steps below before your appointment. These forms help us gather information about your developmental history, communication profile, social interaction, attention, executive functioning, emotional regulation, sensory experiences, and day-to-day functioning.

Step 1: Complete the Adult Developmental History Questionnaire

This questionnaire helps us understand your developmental history, childhood experiences, education, relationships, communication profile, sensory experiences, attention, and daily functioning.

Step 2: Complete the Combined Autism & ADHD Add-On Questionnaire

This questionnaire gathers additional information regarding autism and ADHD-related experiences, including social communication, sensory processing, masking, attention, impulsivity, executive functioning, emotional regulation, and daily life.

Step 3: Informant Information

As part of your combined autism and ADHD assessment, we ask for information from someone who knows you well. This person is referred to as an informant.

Informants help us build a fuller picture of developmental history, childhood experiences, social communication, attention, organisation, behaviours, interests, sensory experiences, and day-to-day functioning.

Suitable informants may include:

  • A parent who can provide childhood developmental history
  • Both parents together
  • A partner or spouse
  • A sibling or close family member
  • A close friend who has known you for many years
  • A combination of a parent and partner where appropriate

Important: For the autism component of the assessment, we will arrange a separate developmental history interview (ADI-R or equivalent).

Wherever possible, an informant who can provide information about childhood development will be required to attend this appointment. This interview forms an important part of the autism assessment process.

The adult being assessed may also attend the interview where appropriate.

Step 4: Adult Neurodevelopmental Informant Questionnaire

Please ask your chosen informant to complete this questionnaire. The information provided will support both the autism and ADHD components of the assessment.

This questionnaire should be completed by the informant and not by the person being assessed.

Step 5: Informant Availability Form (ADI-R Scheduling)

Please ask your chosen informant to complete the availability form below so that we can arrange the separate ADI-R/developmental history interview.

Step 6: Additional Screening Questionnaires

If you have already completed a neurodevelopmental pre-screening assessment with Lycali, you may not need to complete some of the questionnaires below again unless requested by a clinician.

As part of your assessment, you may be asked to complete:

  • Adult AQ-50
  • Adult ASRS
  • Adult SDQ
  • Wender Utah Rating Scale (WURS)

These questionnaires will be sent separately to your email address. Please check your inbox, junk mail and spam folders regularly to ensure you receive all assessment documentation.
